Lutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate and detail-oriented Long-Term Care Functional Screener to join our IRIS Consulting Team in Milwaukee, WI! ๐
In this role, you'll help individuals enrolled in the IRIS Program (Include, Respect, I Self-Direct) determine their eligibility for publicly funded long-term care services. You'll be part of a mission-driven team that empowers people to live independently and meaningfully.
๐ง What You'll Do
- ๐ค Build trusting relationships with IRIS participants
- ๐ Administer Long-Term Care Functional Screens (LTC FS) annually or as needed
- ๐ Coordinate and document LTC FS renewals
- ๐ฌ Provide guidance to help participants maintain funding eligibility
- ๐งโ๐คโ๐ง Collaborate with ICA team members and community partners
- ๐งญ Participate in staff meetings and multidisciplinary conferences
- ๐งโโ๏ธ Prepare reports and may testify in hearings or reviews
- ๐งช Ensure compliance with federal and state documentation standards
- ๐ง Strengthen community connections to support participants

๐ Qualifications
- ๐ Bachelor's degree in social work, psychology, human services, nursing, special education, or related field
- ๐งโ๐ฆฝ Minimum 1 year of experience working with seniors or individuals with disabilities
- โ Valid LTC FS Certification (or willingness to obtain)
- ๐ช Strong commitment to participant autonomy and meaningful living
- ๐ป Comfortable with smartphones, tablets, and computer systems
- ๐ง Knowledge of human behavior, social dynamics, and community resources
- ๐ Strong organizational and planning skills
- ๐ Ability to work independently and collaboratively
๐ Requirements
- Valid driver's license & reliable vehicle
- Satisfactory driving record (MVR check required)
- Must pass Caregiver Background Check
- Willingness to travel frequently (day trips + occasional overnight)
