Adaptable Human Solutions Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our full-time team. Work Environment
This position is primarily remote, allowing you to engage with clients from the comfort of your home while maintaining flexibility in your work hours. Regular working hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional requirements for evenings or weekends based on client needs. Part-time scheduling may be negotiable based on the clinic's demands. You will collaborate with dedicated professionals from various disciplines, including nurses and caregivers, ensuring a team-oriented approach to patient care.
Key Responsibilities
- Conduct comprehensive assessments tailored to seniors' psychosocial needs, focusing on the emotional, mental, and social aspects of their lives.
- Collaborate with clients and their families to create individualized care plans that address their specific goals, preferences, and challenges.
- Provide counseling and emotional support, guiding seniors and their families through transitions and difficult life events.
- Advocate for clients' rights and ensure they have access to essential community resources and support services.
- Work alongside a multidisciplinary team to coordinate care, communicate effectively, and address any issues regarding client safety and well-being.
- Conduct routine client visits to assess their living conditions, monitor well-being, and respond to any emerging concerns.
- Facilitate family meetings to improve communication and resolve any conflicts related to caregiving.
- Educate clients and families about available community programs and support that foster social engagement, help sustain independence, and enhance life quality.
- Maintain meticulous documentation of client interactions, assessments, progress notes, and care plans, in compliance with ethical standards and regulations.
- Stay updated on current social work practices, policies, and regulations to ensure high-quality services.
Patient Population / Client Focus
This role focuses on senior clients, many of whom may be dealing with mental health challenges such as anxiety or depression, as well as issues related to aging and transitions in care settings. You will work closely with both seniors and their families, advocating for their needs and helping them access the resources they require for improved health and well-being.
Qualifications
- A Master's degree in Social Work from an accredited institution.
- Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
- A minimum of two years of relevant experience in social work, with a preference for experience in community settings or geriatric care.
- Knowledge of principles of social work, issues related to aging, and familiarity with community resources.
- Strong assessment, counseling, conflict-resolution, and problem-solving abilities.
- Exceptional empathetic skills, enabling you to connect with clients and foster trust.
- Excellent communication skills for collaboration with interdisciplinary teams.
- Proficiency in using electronic health record (EHR) systems and various computer applications.
Preferred Experience
Preferred candidates will have demonstrable experience working directly with older adult populations in community settings, including nursing homes, independent living facilities, or outpatient services. Familiarity with EHR software and a strong understanding of community health resources are advantageous.
Compensation and Benefits
The compensation for this position ranges from $70,000 to $100,000 annually, commensurate with experience and qualifications.
