Key Responsibilities
- Conduct thorough and compassionate mental health assessments for hospice patients and their families, evaluating psychological needs, coping strategies, and support systems.
- Develop and implement tailored treatment plans utilizing evidence-based practices, focusing on emotional wellness, grief counseling, and familial support.
- Provide individual and group therapy sessions targeted at empowering patients and families to navigate the complexities of grief and end-of-life care.
- Utilize various therapeutic modalities, including cognitive-behavioral therapy (CBT) and trauma-focused therapy, to address specific mental health issues related to terminal diagnoses.
- Conduct regular consultations with an interdisciplinary team, including physicians, nurses, and chaplains, to ensure comprehensive and holistic patient care.
- Educate patients and families about available support systems, and connect them with community resources and assistance programs as necessary.
- Actively participate in hospice care meetings to represent the social work perspective and advocate for the mental health needs of patients.
- Be available for on-call consultations during times of crisis or urgent mental health needs.
Patient Population / Client Focus
Your primary focus will be on hospice patients and their families, who may be dealing with a wide range of emotional challenges, including anxiety, depression, and grief related to terminal illness. Understanding the unique dynamics and diverse backgrounds of these patients is essential to providing meaningful and impactful care.
Qualifications
- A Master's degree in Social Work (MSW) from an accredited institution.
- Current licensure as a Clinical Social Worker (LCSW) in the state of Nevada.
- Strong understanding of hospice care principles and end-of-life issues.
- Excellent interpersonal and communication skills for effective therapy delivery.
- Demonstrated ability to work collaboratively within a healthcare team.
Preferred Experience
- Experience working in hospice, palliative care, or similar settings is highly desirable.
- Expertise in providing grief counseling and support for families undergoing challenging transitions.
- Familiarity with telehealth tools and electronic health record systems.
Compensation and Benefits
